4. APPOINTMENT BOOKING AND SERVICE TERMS
4.1 Booking an Appointment
Appointments may be booked through the Website, by telephone, or by walking into our facility at Ekehuan Barracks, Benin City. When booking online, you agree to provide accurate personal information including your full name, contact number, date of birth, and the test or service required.
4.2 Cancellation and Rescheduling
If you need to cancel or reschedule an appointment, please notify us as early as possible — ideally at least 24 hours before your scheduled appointment time. Failure to attend a booked appointment without prior notification may result in a cancellation fee for certain services.
4.3 Sample Collection Requirements
Certain tests require specific preparation from the patient — such as fasting before blood sugar or lipid profile tests. It is your responsibility to follow the preparation instructions provided at the time of booking or as advised by your doctor. Omokos shall not be liable for inaccurate results arising from failure to follow preparation instructions.
4.4 Home Sample Collection
Where home sample collection services are offered, you agree to provide accurate location information and ensure that an appropriate adult is present at the time of collection. Home collection fees will be communicated clearly at the time of booking.
4.5 Turnaround Time
We aim to deliver results within the timeframes communicated at the time of testing. Turnaround times may vary depending on the nature of the test and laboratory workload. Omokos shall not be liable for delays beyond our reasonable control.
5. PAYMENT TERMS
5.1 Fees
All fees for diagnostic services are communicated clearly before the commencement of any test or service. Payment is required before or at the time of service, unless a formal credit arrangement has been established with Omokos.
5.2 Payment Methods
We accept payment by cash, bank transfer, POS, and such other payment methods as may be made available from time to time. Online payment options available through the Website are secured through approved payment gateways.
5.3 HMO and Corporate Billing
Patients covered under Health Maintenance Organisation (HMO) schemes or corporate health packages must present valid identification and HMO cards at the time of service. Omokos will bill the relevant HMO or corporate body according to agreed rates. Patients are responsible for any co-payments or services not covered by their scheme.
5.4 Refund Policy
Fees paid for laboratory tests that have already been processed are non-refundable. Where a test cannot be completed due to circumstances attributable to Omokos, a full refund or replacement test will be offered at no additional cost. Refund requests must be submitted within 7 days of the service date.
5.5 Price Changes
Omokos reserves the right to revise service fees at any time. Updated prices will be published on the Website and communicated at the point of booking.
6. RESULTS ACCESS AND CONFIDENTIALITY
6.1 Access to Results
Laboratory results and diagnostic reports are confidential documents belonging to the patient. Results may be accessed through the Website using secure login credentials provided at the time of registration, or collected in person at our facility.
6.2 Third Party Access
Results will only be released to third parties — including family members, employers, or insurance companies — with the explicit written consent of the patient, or as required by applicable Nigerian law, court order, or public health authority directive.
6.3 Result Validity Period
Laboratory results are valid at the time they are produced and reflect the state of the patient’s health at the time of sample collection. Results do not remain valid indefinitely and should not be used to make clinical decisions beyond the period clinically appropriate for the test in question.
6.4 Storage of Results
Omokos retains patient records and laboratory results in accordance with applicable Nigerian healthcare regulations and good laboratory practice guidelines. Patients may request copies of their records in accordance with our Patient Records Policy.
